Back pain will be experienced by approximately 50-80 % of the adult population at some point(s) in life. The peak prevalence of these disorders appears to occur between the ages of 55-64 years, and progressively tapers off thereafter. Back pain is one of the most common causes of disability, especially during the productive middle years of life. Primarily because of its prevalence spinal pain produces extensive disability and work loss and thus impacts considerably on the individual and society.

Spinal pain is not always a curable disorder and for many is a lifelong problem. No intervention has been shown to alter the underlying prevalence, incidence or recurrence rates. Passive modalities appear to have no role to play in the management of mechanical spinal pain while there is evidence to support a role for education and exercise.

One of the strongest risk factors for a future episode of back pain is a past history of back pain. Biomechanical risk factors include heavy and repetitive lifting, body

vibration, prolonged or frequent bending and twisting and sustained postural stresses. Prognostic factors include psychosocial, heavy manual work, sitting occupations, presence of sciatica and a lack of centralization.
